Nicely stated, Dr. Qamar. We (the industry and consumers) are in need of clarity of terms here, which you have provided succinctly and accurately. I hope the distinctions that you pose will continue to attach themselves more precisely to the terminology. From my vantage point and experience, I would say that most DPC practices offer some kind of unlimited or nearly unlimited access via phone or email after-hours; many offer 24/7 accessibility. Panel sizes seem to be in the 600 – 800 range, although data shows an extremely broad range of sizes.
